Privacy Policy
The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) requires that medical offices inform patients of their rights, as well as medical providers’ responsibilities, with respects to disclosure of patient’s’ medical records.
The following outlines our policy in that regard:
Our office will keep all of your medical information confidential and will release it for the following purposes (or when authorized in writing by you to disclose it elsewhere):
TREATMENT: Our Registered Dietitians will use information to treat you. Our clinical staff has access to your entire record because often your past medical history is pertinent to your care.
BILLING: Our Billing Office will use the information to bill you. Sometimes carriers request office notes or operative notes in order to process a claim.
APPOINTMENTS AND DEMOGRAPHICS: Our Staff has access to your record for purposes of scheduling your appointments and updating your demographic information in our computers.
Our office will use the information for business purposes such as quality improvement or to contact you or to send information. Because we believe that regular care is crucial for your general health, we will remind you of a scheduled appointment or that it is time for you to make another appointment. Additionally, we may contact you to follow up on your care and inform you of treatment options or services that may be of interest to you or your family.
These means of communication are an important part of our philosophy of partnering with our patients to be sure they receive the best preventative care modern medical nutrition therapy can provide. They may include postcards, letters, telephone reminders, or electronic reminders such as e-mail (unless you tell us that you do not wish to receive any of all of these reminders).
